Program Forms and Information.  2012-13 forms include a medical waiver and code of conduct. 

All players must first complete online registration.

All new players to the DSC Competitive Recreation Program must provide a birth certificate.

If your player resides in NC please complete the Interstate Permission Form and return with $10 to the Club Registrar with your packet.
